HIV Case Management and Prevention

HIV Care Services

HIV Care Services is a comprehensive program designed to assist individuals who are HIV+, or individuals at high risk for acquiring HIV.  The program consists of the following service areas:

Ryan White Care Act Title II Case Management 
Funded by the Ohio Department of Health, Licensed Social Workers focus on on-site HIV case management provision to people living with HIV/AIDS.  Services will include outreach efforts as needed.  The case manager will assist eligible individuals in accessing state funded programs such as:
  • Emergency Financial Assistance Program
  • HIV Home Health Program
  • Ohio HIV Drug Assistance Program
  • Medicaid Spend-down Payment Program
  • Health Insurance Premium Payment Program

HIV/AOD Case Management 
This service, funded by the Stark County Alcohol and Drug Board, offers an added layer of case management services to individuals who are HIV+ and have substance abuse problems.  The case manager offers assistance to individuals in accessing substance abuse treatment and support to them in their recovery. 

HIV Prevention Case Management: 
This program, funded by the Canton City Health Department, offers client centered, systems based, prevention activities that assist HIV positive and HIV negative individuals in adopting risk reduction behaviors. 
 
Geographic Area Served
HIV Care Service regions are defined by the funding sources.  Ryan White Care Act Title II Case Management is provided to Stark, Carroll, Harrison, Holmes, Tuscarawas, and Wayne Counties.  HIV/AOD Case Management is provided to residents of Stark County.  HIV Prevention Case Management is provided to individuals residing in Stark or Carroll Counties. 
 
Hours, Days of Service
Regular Business Hours - 8:00 A.M. to 5:00 P.M. Monday through Friday.  Evening hours can be arranged by appointment. 

Parking is free at the Parking Garage or parking lot with a validated parking ticket. 

Goals:
  1. To make case management as accessible, available and culturally specific as possible for the diversity of individuals living with HIV;
  2. To comply with the Ohio Department of Health’s goal of assuring case management services are available to people with HIV and their families in every county of Ohio;
  3. To assure that all individuals with HIV are able to access physicians and medications per the guidelines developed by the Public Health Service Council;
  4. To provide information/education to people living with HIV regarding transmission, prevention, secondary infection and resistance;
  5. To make HIV+ individuals aware of, and assist them in acquiring all resources for which they may be eligible in order to improve the quality of their lives and as a last resort, approve the use of Ryan White Emergency Financial Assistance funds and/or refer individuals to other Ryan White programs;
  6. To support the substance abuse treatment needs of HIV+ individuals;
  7. To provide specialized assistance to persons with multiple and complex HIV risk reduction needs;
  8. To provide individualized, multiple-session HIV risk reduction counseling to help initiate and maintain behavior changes needed to prevent the transmission or acquisition of HIV;
  9. To facilitate referral services for clients’ medical and psychosocial needs that affect their health and ability to change HIV related risk taking behavior;

Intake/Eligibility Policy
Individuals requesting Ryan White Care Act Title II Case Management or HIV/AOD Services must provide proof of their HIV+ status.  Eligibility for HIV Prevention Case Management services will be assessed as to their need for this service.  Priority would be given to individuals who are at greatest risk of transmitting or acquiring HIV and whose behaviors are not influenced by less intensive HIV prevention interventions.  These individuals have, or are likely to have difficulty initiating or sustaining practices that reduce or prevent HIV transmission due to multiple life issues that pose barriers to changing behaviors. 

Funding and Fee Policy
HIV Services are supported through grant funds.  There are no charges to clients for services.
