Assists senior and disabled adults in arranging and maintaining needed care. Also acts as a connection between seniors and long-distance care-givers and family members. Provides personal care assessments, medical care planning, family education and consultation, emergency assistance and more.

Geriatric Specialized Care Management: Assists seniors in managing monthly finances. Case managers maintain close contact with clients to oversee budgets and ensure financial needs are met. Also provides assistance with home sales, selecting a nursing home, moving and pet care. In addition to Geriatric Care Management and Geriatric Specialized Care Management (listed above), the Family Care Solution Program offers:

Provides help to people in their own home when they are unable to maintain their personal care because of illness, pregnancy, age, job or other situations. Services may include meal preparation, light cleaning, child care (with parent present), respite care, shopping and assistance getting to appointments. 

 

Helps clients with personal hygiene and selected health needs under the supervision of a health care professional. Services may include bathing, dressing, grooming, medication reminder sand more.

 

Provides assistance for medium- and heavy-duty chores, including major cleaning, minor home repair, trash removal and moving heavy objects.
